Wins five consecutive league titles with Dynamo before joining AC Milan for $26 million in 1999.

Factbox on Ukraine striker Andriy Shevchenko who is returning to former team AC Milan from Chelsea, both clubs said on Saturday.

Born: Sept. 29, 1976 in Dvirkivshchyna, Ukraine.

- Comes to the attention of European clubs during the 1997-98 season when he scores a hat-trick for Dynamo Kiev in a 4-0 Champions League win over Barcelona at the Nou Camp.

- Wins five consecutive league titles with Dynamo before joining AC Milan for $26 million in 1999.

- An instant success in Serie A, scoring on his debut in a 2-2 draw at Lecce. Finishes the season as top scorer with 24 goals.

- Spends seven successful years at Milan, being named European Footballer of the Year in 2004. Helps the Rossoneri clinch the 2003/04 Serie A title and scores a penalty in the shootout as they beat Juventus in the 2003 Champions League final.

However, he misses a shootout kick when Milan lose the 2005 Champions League final to Liverpool.

- May 2006: Shevchenko joins English champions Chelsea for an undisclosed club record fee but a string of injuries and a lack of form mean his two years in London are miserable.

- August 23 2008: Chelsea agree terms for his return to Milan although details of the deal are not disclosed.
