Police said the charge of causing manslaughter by gross neglect related to the death of a child in April.
The woman, 22, and man, 36, from the Tyseley area of Birmingham, are due to appear at the city's magistrates' court where they also face three charges of wilful neglect.
Police said the charge of causing manslaughter by gross neglect related to the death of a child in April.
The wilful neglect charges concerned an incident in February and involved three children all aged under four. There were no further details.
The charges come amid the furore caused by the conviction of a mother, her boyfriend and their lodger over the death of Baby P, the 17-month-old toddler who died of abuse despite being on the "at risk" register of Haringey Council in north London.
The government has ordered a review of child protection measures in light of the Baby P case.
